3 chế độ — sensor inputs

1. Forward tilt

  • Chin drops, nose points down
  • Otoliths feel increased forward pitch
  • Semicircular canals register nose-down rotation
  • Visual field shifts to ground few feet ahead

2. Neutral

  • Ears over shoulders, nose level with horizon
  • Calibration point vestibular system expects
  • Canals quiet, otoliths report "up is up"

3. Backwards

  • Chin lifts, nose up
  • Ít gặp, opposite otolith signal
  • Visual field to sky

Vị trí của đầu ở trên thân người = khung tham chiếu. Mọi thứ downstream dùng nó.

Vestibular — bộ dịch đầu tiên

Tai trong không quan tâm ý định. Nó báo gia tốc.

Khi bạn pitch forward:

  • Otoliths shear → báo brainstem "đang tipping"
  • Signal splits:
  • Branch 1: tới VOR
  • Branch 2: tới postural centers

VOR — mắt cố giữ thăng bằng

VOR = Vestibulo-Ocular Reflex: automatic, xoay mắt ngược chiều đầu để hình ảnh không nhòe

Khi đầu pitch down 15°, VOR lái mắt lên ~15° để bù.

Thất bại với sustained forward tilt

  1. Eyes hit orbital limit (không thể nhìn lên mãi khi chin stay down)
  2. Visual input changes — horizon leaves upper visual field

"Forward tilt = no more horizon" — literal, không phải thơ Mất horizon → brain down-weights vision, up-weights vestibular + proprioception

Mắt tới não — bottleneck tích hợp

Brainstem + cerebellum so sánh 3 maps:

  1. Vestibular: head is down
  2. Visual: ground texture moving, no stable horizon
  3. Somatosensory: pressure under feet, stretch in calves

Khi vision degraded

  • Hệ thống trở nên thận trọng
  • Thay vì cho phép ankle strategy (small, fast adjustments) → chọn stiffening strategy
  • Đó là "no bend of the knees"

Không phải laziness. Locking knees giảm degrees of freedom → giảm sensory processing cần thiết Cảm giác safe short-term, nhưng less able to absorb perturbation → why beginners trên skis look rigid

Neutral head:

  • VOR gain normal, eyes stay on horizon
  • Brain trusts vision → permits knee + ankle flexion
  • Fluid, anticipatory adjustments

Sustained forward head:

  • VOR working overtime but visual anchor gone
  • Brain reduces trust in vision, increases co-contraction
  • Quadriceps + calves stiffen, knees extended, hips shift back

Đây là lý do coaches cue "head up" trước "bend your knees" Bạn không thể có gối cho tới khi đầu cho não phép

Rehab

  • Gaze-stabilization exercises (×1, ×2 VOR training) restore head-eye link
  • Patients stop locking legs

Sport

  • Snowboarder looking at board → lose horizon → stiffen → catch edge
  • Fighter dropping chin to watch feet → lose peripheral vision + balance

Daily life

  • "Phone-neck" posture = chronic forward tilt
  • Over time system adapts to no-horizon condition
  • One reason people feel unsteady when finally look up

Kết luận

Balance training starts at the skull, not the feet.

Thay đổi node đầu tiên — bring nose back to neutral, keep horizon ở 1/3 trên visual field — phần còn lại của chain tự tổ chức lại mà không cần bạn nghĩ "bend knees".
